By default, the SmartCentral application form includes mandatory document requests such as birth certificates and immunisation records. However, there may be instances where you require additional documents from parents that are not part of the standard application. We enable you add supplementary document requests into your application form and you can choose to mark these documents as either mandatory or optional.
Follow the steps below on how to request families provide specific documents :
- Click Account from the top menu bar and click Setup org from the dropdown menu.

The uploaded file appears on the top of the order and a notification appears at the bottom of the page.
