Adding Supplementary Questions to Application Forms.

There may be instances where you require additional information from parents that is not part of the standard application form. We enable you add supplementary questions into your application form and you can choose to mark these questions as either mandatory or optional.

Follow the steps below on how to request families provide additional information:



  1. Login to the system, click Account from the top menu bar.

  1. dropdown menu appears. Click Setup org.


 Note
: the ORANGE coloured button does not relate to services, May/may not appear on the dropdown menu.

 

  1. Edit organisation page appears. Click on the Questions tab.

  1. A blank question template appears at the bottom of the page. Scroll down to the bottom of the

page and enter Key details.

  1. On the Key text field, enter “Sunscreen Consent” the Key is used as an internal question name.
    Note: “Label to Display”  automatically displays the entered value on the Key text field, but can be changed 
    to the correct label as it appears on the enrolment form.
    Recommendations: Best practise to use 
     “sunscreen_consent” or one word sunscreen

 

 

  1. Enter the description. The description text field includes the question text displayed on the form.

  1. Select from the question type dropdown menu:

    Text - a single line of text
    Yes/No - a dropdown with options Yes and No
    List - a customisable dropdown pick list
    Since the question we used is answerable by Yes/No, click to select yes/no

This set up may vary depending on the answer required from the family on the enrolment form.  Do not provide an answer, answer only by selecting “yes”/“no”. If the answer requires the family to provide details, the text should be selected, or if the family is required to pick an answer from the list, then the list should be selected.

  1. Set the visibility of the Question:

     

Everywhere - questions will appear on the enrolment form.,
Except waitlist - will not appear on the waitlist. Generally used
 when a waiting list profile is created
Staff Only - required mandatory or c
ommonly used when staff may require specific information and would like it to be included in reporting.
 

 

  1. Once completed, click Preview on the top of the page to see the preview of the added Question.

 

 

  1. Preview window appears. Preview allows services to see how all questions will appear on the enrolment form. Scroll down to the bottom of the page, see the preview of the added question, which includes a yes/no dropdown menu.
    Note:  To continue customising the question, scroll up to the Preview window and click the X icon on the top right corner. 

 

  1. When happy with the question entered, click Publish to save all entries.

A notification appears on the bottom when the question is successfully published.
