After the template has been added to the Forms, it can be configured. Follow the instructions below on how to configure the Forms:
1. Login to Smart Central. Then click Account from the top menu bar. Select setup org from the dropdown menu. |
2. Edit Organisation page appears. Then, click Forms |
3. Form Templates page appears. Then, click Configure |
4. The questions page appears. Then, click Add |
A row to add questions appear. Set the following details:

5. Click Publish to save the entries. a notification appears on the bottom. |
6. Go back to Services> select Centre> Forms to see the published view. Select the published form from the Forms dropdown menu and click Add. |
7. Preview of the form appears. Keep adding and configuring the questions as desired. |
